ABOUT ME

I am honored that you are at Elevation of Care.  My name is Shantrael Taylor,  I am a Mom, Advocate and Transition coach of Narrative Medicine.  I am excited to share with you my new journey . After much time with God and self work I am moving further into my purpose.  My new journey is to be a beacon of hope in dark times, a resource and educator as a Transition Coach.  In 2015 at the age of 5, my son Darren, may he forever rest in peace, suffered a Tramuatic Brain Injury.  At that moment we were thrust into a new way of life post brain injury.  I needed help, I needed tool, I needed a transition coach but there were none.  I learned to navigate the ins and outs of the special needs community and championed for Darren to still have the most beautiful life.  In 2020, I gave birith to the most resilient human I know Jaxon.  Jaxon's delivery was very traumatic.  He suffered a Hypoxic brain injury resulting in HIE and a host of other diagnoses.  With the tremendous amount of expertise I have, I now help medically fragile and complex care family units transition from hospitals and facilities to home.   For seven years I have watched the gap between home care and hospital care lack the information and tools to go home successfully.   Specfically, I noticed the gap in being equipped with adequate understanding and knowledge of how to support and care for children in NICU, Traumatic Brain Injury, and evern complex care of the Aging Community.  It requires a new level of care and I want you to feel empowered Knowledgeable and Confident to provide the kind of care while nurturing yourself through the process.

I guide families, care teams, and medical professionls through successful transition of care from a mdical facility, new diagnose, and life changes.   I provide tools strategies, and guided support in the form of classes, workbooks, videos, workshops,and webinars all with a playbook I've created.  I know this needs to be in the hands of caregivers like me who need this level of support as they are navigation such pivotal moments in their lives.  To lean more about my story visit www.godsaidnollc.com
