Elevation of heart is a journey of my heart and my life.  As a parental caregiver I was often times looking for help and resources and none were available transitioning from hospital to home; old normal to new normal; summer vacation to school year...you get it right...CHANGE!  I needed help navigating change and transition however there was nothing out to help bridge that gap.  Well, its been created and is what we know now as Elevation of Care.
